Job Title:DIS Case Management SpecialistLocation: Indianapolis, IN 46204Duration: 12/31/2022Hours: 7:30am - 4:30 pmThe DIS Case Management Specialist duties will include but are not limited to the following: monitoring of case management and partner services performance through quarterly pouch audits and quality assurance reviews, serving as the liaison between the STD Program and District as needed, hosting check-ins on a weekly/bi-weekly basis with Area Managers/Supervisors, assisting in the development of performance measures for the evaluation of Partner Services and DIS activities, assisting in the development of standardized policies/procedures. This position will be required to assist in completion of federal grant deliverables as needed. This position will assist in the monitoring and surveillance of STD investigations in the state of Indiana.Completion of a training course entitled Sexually Transmitted Disease (STD) Passport to Partner Services and achievement of a grade of 80% or higher on all examinations as required by the Centers for DiseaseControl and Prevention (CDC) within 6 months of hiring. The STD Passport to Partner Services is a series of online modules, each dedicated to a specific topic in STD and HIV prevention and diseaseintervention/investigative action. Additionally, the completion of all required and assigned trainings as provided by the STD Program's DIS Training Coordinator.Completion of an approved Authorization Training course, whether that training course be provided by IDOH or a federally funded course from the CDC or other regional partner.Successful completion of the HIV Skills Building Course for DIS, as provided by the STD Program, which included observed testing and referral sessions. The DIS may complete the HIV Prevention Program's training course, as provided by the HIV Prevention Trainer, as well.Completion of the Area Manager Case Review Training, as provided by the STD Program, which will be followed by successful completion of syphilis case reviews, as detemined by the STD OperationsManager.Job Requirements:Extensive knowledge of and the ability to interpret federal, state, and local laws, rules, regulations, and policies pertaining to HIV and STD infections and surveillance activities.Extensive knowledge of record management, problem identification/solving methods, consultation methods, and the ability to apply these to HIV and STD Prevention and Surveillance activities.Ability to effectively and appropriately represent the Program, the Division, and/or Agency at local, state, or nationwide events regarding population health and public health.Ability to work effectively with external organizations, individuals, health care professionals, and clinic staff.Ability communicate effectively verbally in-person or over the phone with direct team members, division members, and external partners. Ability to communicate effectively in writing and in an appropriate style for government documents.Ability to work with and maintain confidential information and security of HIV and STD surveillance data.Ability to learn about state and federal departments and agencies which have programs or services working with and for HIV and STD Surveillance.Ability to learn and use Microsoft programs, including but not limited to Outlook, Teams, Excel, Word andPublisher. Additionally, ability to use programs such as Adobe Acrobat, and videoconferencing services such as Zoom or WebEx.Ability to travel throughout Indiana, sometimes on emergency notice, including overnight travel.Ability to elicit sensitive information from patients with STDs or HIV regarding lifestyle choices (including substance use), sexual practices, and barriers faced to safer sexual experiences in a non-judgmental manner.Ability to discuss with medical providers and medical staff the CDC's recommendations for treatment and prevention and appropriately advise on these subjects in regards to a patient's STD care. Additionally, the ability to appropriately advise grantees and their sites on the CDC's recommendations for treatment and prevention.Ability to work independently and be self-sufficient as well as the ability to work within a team on shared tasks, projects, or assignments. Additionally, the ability to provide appropriate report of activities completed independently.Ability to effectively advise grantees and direct reports by providing constructive, positive feedback and engaging in growth-driven performance review with districts.Ability in managing time effectively and the ability to maintain organization throughout all projects and tasks.Ability to engage in professional development education and trainings.Ability to be manage and adequately work on multiple high priority tasks/projects at one time with varyingor simaltaneous due dates. Ability to effectively manage multiple projects or tasks that may have a variety of work methods.Ability to work in a fast-paced, evolving environment and the ability to synthesize and integrate new, evidence-based public health research and models.Applicant must possess as B.S. degree from a four year accredited college or University public health or social sciences; or six years professional experience in public health or a health care related field.Applicant must possess a valid Indiana (or other US State) driver's license.Ability to work, communicate and provide services to patients and clients of culturally, racially, ethnically, economically and geographically diverse populations in a non-judgmental, empathetic manner in safe and comfortable environments.Applicant must have the ability to work with culturally diverse populations in a non-judgmental environment.Contracted position for the IDOH STD Prevention Program.
