Nurse Case Manager

Job Description: Duties and Responsibilities: Plans and performs specialized and advanced medical case management services Oversees preparation, advises and reviews all packets for accuracy and completeness prior to signing and forwarding to the next level in each system Responsible for the preparation and management of Reserve Component Managed Care (RCMC) cases Requirements: Licensed Vocational/Practitioner Nurse (LVN/LPN) or Registered Nurse (RN) Minimum three years experience related to Military Health Care Knowledge, experience, and skills in the area of providing case management to patients and their families while assisting the patient's social adjustment Must be proficient in MS office suite and general administrative duties Be a US Citizen and possess a valid driver's license Successfully complete a government background check Comply with employer's work standards and policies Equal Opportunity Employer Veteran/Disabled
