Discharge Planning Specialist LOH

The Discharge Planning Specialist at Linden Oaks, provides care coordination and assists with discharge planning services to patients who are in need of ongoing behavioral health consults in the EEH emergency departments or behavioral health inpatient care following discharge from the emergency department.

Coordinates Psychiatrist consults and discharge plan at the direction of the patient’s treatment team. Obtains appropriate releases, communicates with other care providers / hospitals and collaborates with patients and families and the treatment team to arrange and document a comprehensive discharge plan.

Required Education, Skills and / or Experience :

Bachelor’s Degree in Social Work or a behavioral health related field

A minimum of one year of experience in a behavioral health care setting, preferably acute care.

Preferred Education, Skills and / or Experience :

A minimum of one year of previous social work experience
