OverviewWork Hours: Full-time 8a-4:30pDescriptionJOB DESCRIPTION:Analyzes medical records for content ensuring that all required components are present per record type per regulatory requirements.

Work reports to reduce the unbilled/pending accounts.

Maintains, updates, verifies, distributes, and retrieve patient information in accordance with organization standards and regulatory requirements.

Retrieves records from patient care areas; perform discharge reconciliation to ensure all records are retrieved from patient care areas in a timely manner; scan and prep medical information into the appropriate patient folder within power chart.

Inform immediate supervisor of any missing discharges within 24 hours of discharge.

May act as a backup for Vital Record Coordinator.

May monitor transcription queues.

Retrieve records from patient care areas; prep and scan medical documents into the correct event set within the appropriate encounter; assist customers.

Enters discharges being sent for scanning in Overlook Management System, and print barcode coversheet for each discharge being sent for outside scanning.

Analyzes records (discharged or in-house) for content per record type.

Insures that the healthcare practitioners/medical staff are informed of missing components in a timely manner to promote record completion and compliance with state and federal guidelines.

Enters data into the Profile Record Management tool to track deficiencies and medical staff compliance with completion requirements.

Complete birth certificate for each live birth using electronic birth certificate software; process request for paternity acknowledgement upon patient direction; submit birth certificate to Vital Records Office electronically; provides parents with confirmation of birth letter with instructions on how to retrieve official birth certificate from Vital Records Office; scan confirmation of birth letter into newborn record; update infants name in Health Quest after discharge.

Monitor transcription queues to ensure all rejected reports are processed in a timely manner.

Performs quality review of analyzed records insuring that the medical practitioners are notified of missing or incomplete required components.

Performs quality review of every scanned document prior to image acceptance.

Perform a second review of deficiencies entered into HIM profile to ensure the appropriate physician is being tagged for completion.

Inform immediate supervisor of any new documents that are not listed on forms inventory; reviews event set matrix to ensure documents are being scanned into the correct folder.

Attend regional vital records meetings to continue knowledge for birth certificate completion.

Follow Medical Staff Rules and Regulations to ensure analysis function is being done in accordance to guidelines.

Identifies and/or investigates record content issues that identify patient documentation errors.

Works with other department peers.

MINIMUM QUALIFICATIONS:High school diploma or equivalent.

Three years of experience in Health Information Management in acute care setting.

Three years of experience working with Electronic Health Record (EHR).
