We facilitate the centralized communication and coordination of patient movement across the entirety of the St. Luke’s Health System, as well as the admission of patients to/from outside facilities. In addition to coordinating complex patient consultation and placement, the Transfer Center staff are authorized to enact time-sensitive patient algorithms to promote acute and necessary patient care transitions. Operating 24/7 with both clinical and nonclinical personnel, the Transfer Center performs a vital service for patients throughout the state and region. Advanced critical thinking skills, mature ability to form positive relationships and a passion for innovative care models is essential. The Registered Nurse is responsible for evaluating patients and assisting physicians with patient care. Plans, implements, and documents nursing care based on patient’s needs.Schedule: 12hr shifts; this position is for nights 1900-0700; rotating weekend and holidays.This position does require adept typing skills and is not remote .Responsibilities
Identifies patient needs and determines priorities for care through assessment, planning, intervention, and evaluation.
May be responsible for overseeing and coordinating the transport and transfer of patients between facilities prioritizing transport urgency and level of clinical care needed.
Requires substantial specialized knowledge, judgment and skill based upon the principles of biological, physiological, behavioral, and sociological sciences of all patient populations.
Develops, coordinates, and implements daily plans for patient assignment, which accommodates patient needs, nursing unit workload, and physician preferences.
Uses advanced critical thinking skills in making decisions regarding all aspects of patient care. Provides patient centric care in alignment with professional nursing practice model.
Manages the care of patients in a variety of settings both internal and external to the organization.
Advocates for and employs evidence-based practice care standards and supports adoption of evidence-based practices and innovations in care delivery.
May assist with the initial evaluation of patients for appropriateness of admission type and setting, support of patient care team, escalation and/or delegation of tasks and patient needs and patient education regarding the role of the telehealth/transfer nurse.
Documents care based on standards of care and practice, patient acuity, and/or clinical priorities.
Effectively utilizes a variety of technologies to meet the needs of our patients, providers, facilities and other entities.
May assist with the development and mentoring of bedside care teams.
Other duties and responsibilities as assigned.
Qualifications
Education: Bachelor's of Nursing Degree
Experience: 3 year’s clinical RN experience
Licenses/Certifications: Current RN licensure in the State(s) where care is provided
