JOB SUMMARY

Care Coordination is an essential element of the agency's population health work. The care coordinator contacts patients with care gaps to schedule and complete needed visits. The Care Coordinator is responsible for helping patients keep established visits and problem solves when there are identified difficulties in keeping those appointments. The care coordinator is well versed in community support services and will ensure patients who need such services are connected to them (e.g., transportation). The CC may attend Integrated Care Team meetings to share findings and suggestions from SDOH screening calls. The Care Coordinator works collaboratively with other members of the health care team as necessary including the primary care providers, medical assistants, behavioral health providers, nurses, medical assistants, Pharmacists, Access to Care staff and other ancillary staff as well as outside agencies.

ROLE AND RESPONSIBILITIES

  • Identifies those patients who are need of scheduling well care visits from the Share Point dashboard report
  • Maintains a tracking system to monitor the scheduling attempts and appointments
  • Completes SDOH screening in advance of ICMs to support patient engagement in care
  • Utilizes innovative modes of communication to engage patients in committing to appointments and problem solving with them as needed
  • Assists staff to identify any "navigation" issues to ensure that patients they are able to attend scheduled medical visits both at CHC and outside the agency
  • Coordinates with the Operation Managers to schedule patients as needed
  • Resolves challenging issues of community care coordination and resources that cannot be managed by Access to Care staff
  • Contacts patients on value based rosters who have never been seen at CHC to establish them as patients
  • Completes other population health projects as needed

QUALIFICATIONS

Required Skills and Education

  • Bachelor's degree BA/BS preferred
  • Knowledge of and commitment to patient population/community resources
  • Bi-lingual in English/Spanish strongly preferred
  • Ability to work independently and collaboratively
  • Knowledge of healthcare delivery, clinical practice, and healthcare operational support
  • Excellent interpersonal, written and oral communications skills
  • Superior organizational skills and attention to detail
  • Proven ability to multi task and meet established deadlines
