Position Description: To provide medical supervision and meaningful educational activities to one adult with Developmental Disabilities in the home of the individual.

Sign-On Bonus: PPCH will pay up to $700 for sign on bonus after hiring. Employees will receive up to $350 after the first 30 days and the remainder up to $350 after 60 days as long as you are still employed. Since this is a part time position/part time employee, the bonus award and retention payouts and payment will be pro-rated based upon the percentage of hours that make up a full time work week.

Retention Bonus: Compensation will be provided in the form of payment for a specific approved amount. PPCH will pay up to $1150 for an employment retention bonus. Employees will receive up to $500 after 6 months of employment and the remainder up to $650 after 12 months of employment as long as you are still employed. Since this is a part time position/part time employee, the bonus award and retention payouts and payment will be pro-rated based upon the percentage of hours that make up a full time work week.

Part-Time: 25 hours per week, Monday thru Friday (5 hours per day). There is an option for Full-Time, if you're wanting full-time hours. Please ask our Recruiter for further details.

Starting Pay: $16 per hour

Location: 112th Avenue & Federal Boulevard (Westminster, Colorado)

Position Duties:

Assist the customer with all activities of daily living; catheter care, gastrostomy feedings, medication administration, tracheostomy care, hygiene, etc. (this will include physically transferring customer from wheelchair to changing table).

Establish and follow an approved schedule of appropriate activities and ensure the customer is treated with dignity and respect at all times.

Provide daily documentation, as per PPCH policies and the procedures and rules/regulations of the Division for Developmental Disabilities.

Follow all customer’s Individual Service Plan and health care plans as directed by the physician

Ensure the safety and well-being of the customer throughout the day by providing proper supervision and assisting with safety skills.

Provide ongoing communications with Director of Day Program Services, nursing department, team members, family members and all other stakeholders regarding any issues, concerns, problems (i.e. programmatically and medically).

Observe confidentiality of customer and PPCH operations.

Maintain competency with agency policies and procedures as well as those outlined by the Department of Health Care Policy and Financing

Exhibit professionalism and appropriateness at all times.

Attends and show competency in all PPCH training and meetings.

Qualifications:

One year direct care experience serving individuals with acute and significant medical needs, including catheter care, gastrostomy tube and tracheostomy care.

Basic knowledge of medical terminology.

Knowledge of medical protocols, diagnoses and medications

Experience in the health care field preferred

Good oral and written communication skills

Professional attitude and teamwork mentality

Strong ability to self-direct and use critical thinking

Outstanding attendance and strong work ethic

Current certification in CPR and First Aid

QMAP Certified (as provided by PPCH)

Current/Valid Colorado Driver’s License

Good driving record that meets the requirements of agencies insurance carrier

Ability to pass criminal background checks

Ability to prove the right to work in the U.S.

Reliable transportation.

Minimum Physical Requirements and Working Conditions:

Must be able to lift approximately 75lbs.

Requires repetitive lifting and carrying of approximately 15 lbs. as it pertains to customer care items.

Must be able to push/pull a wheelchair with a customer of moderate weight.

Must be able to physically assist customer with transfers from wheelchair to changing table, toilet or car seat as needed.

Requires moderate amount of reaching over head and away from body

May be required to bend, stoop, squat and climb as it pertains to assessing an environment.

Requires staff to sit for periods of time, walk for up to several hours at a time with breaks

Work is primarily indoors
