We support Denver’s homeless veterans who have been underserved, and prepare preschoolers challenged by poverty to be successful in elementary school and beyond. Wherever we go we engage faith, relationship-building, and volunteers to lift-up and support Colorado’s most vulnerable citizens.JOB SUMMARYThe Healthy Aging Coordinator is responsible for providing motivational interviewing to program participants, maintaining program records and data, monitoring all service reports and billings and the development of health education, promotional materials, and newsletters. Participates in planning, implementation, and evaluation of the program to ensure efficient delivery of service.ESSENTIAL DUTIES AND RESPONSIBILITIESCommunicates concepts of the Healthy Aging programs to participants, host agencies, other community groups and ensures compliance with federal guidelines and local project policies and procedures.
Monitor client progress with weekly coaching calls to program participants as follow up to established fitness goals and plans.
Maintains and ensures confidentiality of all program participant information.
Develops health education and program promotional materials.
Develops the newsletter for the Healthy Aging program.
Completes in-home pre and post assessments with program clients.
Instructs other Healthy Aging classes as needed.
Provides staff support to the Healthy Aging program staff and partner agencies.
Assists in preparing data and reports for the Healthy Aging programs.
Maintains budgeted service unit records for all Healthy Aging programs and reports monthly DRCOG grant units to accounting for reimbursement submission.
Predictable, consistent attendance.
Performs all other duties as assigned.
RequirementsMINIMUM QUALIFICATIONSAssociate degree in gerontology or related human services/ fitness/dietary field or equivalent relevant experience.
Strong computer skills in Microsoft office.
Own transportation, valid Colorado driver’s license, and proof of insurance.
Preferred Qualifications of PositionExperience in exercise prescription preferred.
Certification as a personal trainer helpful.
KNOWLEDGE AND SKILLSExcellent verbal and written communication skills.
Detail oriented with excellent organizational and record keeping skills.
Knowledge of computer operations Excel, Publisher, Microsoft Word, and databased programs.
WORKING CONDITIONS AND PHYSICAL REQUIREMENTSWhile performing the duties of this position, the employee is regularly required to talk or hear.
Travel utilizing personal vehicle will be required.
Mild physical activity is required while instructions of Healthy Aging classes are offered.
Meet with program participants in their homes or other designated locations.
The noise level is usually low to moderate.
PAY$18.00 - $19.50/HOURPOSITION TYPE AND EXPECTED HOURS30 hours per week with benefits
