Santa Barbara Neighborhood Clinics is a non-profit organization that treats mostly under-served and uninsured patients at our four medical clinics and dental clinic. We currently have an opening for a Licensed Clinical Social Worker. Fluency in English and Spanish is required. We are looking for enthusiastic people who can be effective in a fast-paced setting and provide outstanding customer service.

SUMMARY:

The Licensed Clinical Social Worker (LCSW) is responsible for providing and coordinating behavioral health services for patients as part of an integrative model of care within the primary care setting. The LCSW collaborates with the primary care and behavioral health staff as well as other internal and external service providers in order to effectively manage mental health and substance use conditions and support physical health outcomes. This position is an integral part of the Patient Centered Medical Home (PCMH). LCSW will support Integrated Care Coordination between all SBNC disciplines.

Santa Barbara Neighborhood Clinics has entered a collaboration with Santa Barbara with the Housing Authority of the City of Santa Barbara. This grant will be referred to as HACSB Grant. This position will manage patients on site with the assistance of a Registered Nurse, Patient Access Navigator/Wellness Navigator and an LCSW. Independent judgment is required to plan, prioritize and organize diversified caseload with professionalism and discretion. The position continually requires demonstrated poise, tact and diplomacy in working with a diverse group of internal and external partners.

This position specifically supports the development and provision of trauma-informed care services across the lifespan.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Crisis Stabilization Services will be triaged by the Registered Nurse Care Manager and assessed by the Licensed Clinical Social Worker for mild to moderate cases. Serve cases will be referred for specialized treatment to the County’s Crisis Stabilization Unit, Santa Barbara Cottage Hospital, County of Santa Barbara Behavioral Wellness or Sanctuary Centers of Santa Barbara. All of these providers have Memorandum of Understanding (MOU) with SBNC.

Mental Health Services will be provided by the LCSW and if expanded service are necessary will be referred through existing MOUS to Sanctuary Centers of Santa Barbara or New Beginnings

Life Skills Training will be provided by the LCSW.

Alcohol and Drug Abuse Services will be provided by the LCSW with referrals to SBNV’s Bridge Clinic. Provide billable patient care services in accordance with organizational productivity standards. This includes: individual and group therapy as well as supervised visits provided by MSW, BSW social workers and interns.

Identify parent/patient problem areas and strengths/support networks; develop treatment plans, and document patient progress in accordance with established standards.

SBNC will coordinate transportation with CenCal Health Monitor patient progress with goals and provide appropriate follow-up services.

Provide linkage to services and coordinate communications between internal and external service providers.

Plan schedules to assure the ability to address emergent issues in a timely manner.

Obtain appropriate consultation with supervision as needed.

Complete written documentation according to program standards and timelines in the electronic health system.

Participate in individual and group supervision as required.

Attend staff meetings and trainings as required.

Counsel patients in individual and group sessions to assist in recovery from behavioral health issues, including mild to moderate mental health diagnoses and chemical dependencies.

Utilize consultation data and social work experience to plan and coordinate patient care and rehabilitation, following though to ensure service efficacy.

Counsel family members to assist them in understanding, dealing with, and supporting the patient.

Modify treatment plans to comply with changes in clients’ status.

Monitor, evaluate and record patient progress according to measurable goals described in treatment and care plan.

Identify environmental impediments to patient progress through interviews and review of patient records.

While performing the duties of this job, the employee is required to frequently sit in upright position at a desk for office work.

Work Conditions:

Employee will travel to and from the Housing Authority’s Units we are currently engaged. You may have a group session or a one-on-one counseling. This may result in exposure to variable weather conditions.

Indoor medical office setting with limited space.

Air conditioned in summer and central heat in winter.

Employee may have occasional exposure to moisture, unpleasant odors, and dust, dirt, cold, biological and chemical agents. Employee may have occasional exposure to moderate noise levels consistent with business activity in a busy medical office setting.

Work Hours:

Eight-hour work days with variable/flexible hours depending on patient care needs. Overtime may be required but as an exempt employee you will not be paid for any extra hours.

QUALIFICATIONS AND EDUCATION REQUIREMENTS:

Education: Master of Social Work (M.S.W.) Degree from an accredited University. Current California licensure as a Licensed Clinical Social Worker (LCSW).

Experience: Five years’ experience in clinical social work

Knowledge of: Human behavior and performance, individual differences in ability, personality and interests, learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders. Methods, principles and procedures for diagnosis, treatment and rehabilitation of physical and mental dysfunctions, and for career counseling and guidance.

Ability to: Fully engage with people, taking time to understand their needs through well-established interview techniques. Identify complex problems and review related information to develop and evaluate options and implement solutions.

Physical Requirements: Sitting for an hour at a time with limited breaks between clients. Using computer keyboard or touch screen to enter patient information into EMR system.
