Responsibilities

1. 1. Maintain confidentially, protecting patient information at all times : minimum information necessary to those with right and need to know.

2. 2. Demonstrates understanding to organize, sort the medical / business records and prep the record by removing staples, dividers, taping down loose sheets, counting pages and scanning them appropriately

3. Verifies each page of the record is in the appropriately labeled folder by patient name, MRN, & volume number, in assuring a complete, legal medical record.

4. 4. Accurately accesses the completeness of the legal medical record, with correct assignment of tracking or identification records from record boxes.

5. 5. All reports are reviewed, and any identified discrepancies are addressed.

6. 6. Accurately accesses the medical records, and identify with discharge date and communicate with the medical staff member for timely and accurate chart completion.

7. Ensure accurate pull of all medical / business records according to department types.

8. 7. Performs as a liaison, in accurate & timely chart completion through collaborative methods.

9. 8. Timely update of deficiencies, accurate update on deficiency sheet & in deficiency system, and proper forwarding of the medical record to the correct location in the re-routing process.

10. 9. Oral and written communications are clear & concise, affecting efficient assistance to respective department as necessary.

11. 11. Monitors warehouse inventory system for delivered and undelivered records; identifying and placing them into the appropriate place in a timely manner.

12. Follows policy and procedure for maintaining and retrieving Medical / business records when a release of information requests arises.

13. Other duties as assigned.

Qualifications

EDUCATION, EXPERIENCE, TRAINING

1. High School Diploma or equivalent required

2. 1 year Medical Record Storage or scanning experience preferred

3. Completion of a medical terminology course; preferred

4. Knowledge of terminal digit filing and medical terminology; preferred.

5. Accurate alphabetic, numeric, and / or terminal-digit filing; preferred.

6. Knowledge of State & Federal regulatory requirements for medical staff documentation; preferred.

7. Computer data entry with 10-key, with accurate typing speed of 35 wpm; preferred.

8. High degree of self-motivation, commitment and integrity.

9. Strong computer skills (MS Office)

10. Valid driver’s license preferred.

11. Travel up to 20% of the time may be required.
