Behavioral Health Patient Navigator (Case Manager)

Position Summary: Behavioral Health Patient Navigator (Case Manager) provides case management services to patients being served by the Behavioral Health program. Patient Navigators are committed to removing the patient’s barriers to care by identifying critical resources for patients, helping them navigate through health care services and systems, and promoting patient health. They work closely with the Care Team, which may include behavioral health providers, medical providers, nurses, nurse case managers, and other clinical staff to support positive patient health outcomes.Work Experience: Minimum of two (2) years case management or patient navigation experience required. Experience in behavioral health, primary care or public health highly desirable.
