VCC is an equal opportunity employer.ResponsibilitiesProvide case management and prenatal education to individuals and groups of prenatal patients receiving services at Vista Community Clinic. Rotate through all prenatal service locations as needed.Utilize OB triage protocol to conduct triage on patients who call or show at the clinic and schedule patients for appointments as appropriateConduct patient “checkout” to ensure all requires services were received and that all recommended follow-up appointments and referrals are completed.Prepare package and complete intake on new patients following established CPSP and VCC protocolsComplete and maintain assessments for health education, nutrition and psychosocial CPSP componentsProvide case management services to clients including coordination with client, family members, client support systems and members of the interdisciplinary teamProcess necessary referrals and coordinate necessary appointmentsQualificationsMinimumHigh school graduate or equivalentMinimum one year clinical experience (MA) or experience in perinatal care with basic knowledge of Prenatal care for women who are pregnantCPR Certification (BLS Child, Infant, Adult)Bilingual English/SpanishValid California driver’s license, current vehicle insurance, and reliable transportation to travel between sites for meetings and/or to gather information for reporting purposes; minimum two years’ driving experience; with no major violations on driving recordPreferredMedical Assistant certificate Minimum three years’ maternal and child health experience, including direct patient servicesRequired Skills/Knowledge/AbilitiesExperience with a Medi-Cal program in an outpatient settingBasic computer knowledge, internet use, MS Office and data entry skillsPay Range$22.00 - $26.00PI238379248 Apply
