POSITION SUMMARY:

Under general supervision, the Rehabilitation Specialist provides support services including vocational training and education to individuals with moderate to severe physical and behavioral disabilities in community or worksite settings. Employees in this job class may, evaluate general well-being of individuals with disabilities, teach living skills and assist in enhancing the overall health and well-being of the individual. This job class requires supervisory skills in managing individuals with disabilities, and the ability to oversee daily activities, document and maintain individual files, and patiently caring for individuals with disabilities.

TYPICAL DUTIES:

1. Monitors and teaches individuals with disabilities job tasks and/or educational tasks both in community and worksite settings.

2. Provides care and direction for individuals with disabilities. Depending on the needs and abilities of the individual, the care and direction may include personal hygiene, feeding and proper nutrition, meal planning and preparation, self-help living skills, job application skills, school or work activities, cleaning work/living area, social skills, behavior management, and safety.

3. Assists individuals with arrival and departures; meeting and monitoring public transportation, caretakers, etc.

4. Provides direction and supervision for work or school related activities. This may include items such as; safety, quality control, and other items related to work or school.

5. Communicates with appropriate parties on information relating to the employee’s health, behavior, and well-being.

6. Maintains case files to include case notes, incident reports, health and safety risk assessments, and related paperwork on a daily basis.

7. Plans, and supervises individuals with disabilities on community outings and experiences.

8. Performs other duties and special projects as assigned.

DenotesEssential Job Function

MINIMUM QUALIFICATIONS:

Entry level to five years of experience.

Experience working with people with disabilities preferred;

Ability to communicate effectively and respond to questions and requests;

Effective written communication skills using appropriate business English;

Computer literacy to use business software, the Internet, enter data/retrieve data;

Human relations skills to build effective working relationships;

Demonstrated customer service, problem solving and common sense skills.

EDUCATION REQUIREMENTS:

High School Diploma or GED

CERTIFICATES REQUIRED: 

The following licenses or certificates may be required depending on local, state and/or contract requirements:

CPR/First Aid

Valid Driver's License

PHYSICAL REQUIREMENTS:

Employees must have the ability to perform the following physical demands for an extended period with or without assistance.

Maneuvering in and around worksite

Lifting to 30 pounds

Viewing computer screen

Utilizing keyboard 

WORK ENVIRONMENT:

Work is performed in a business office with limited privacy and exposure to background noise. Employees will work with clients/employees with behavioral challenges. Work may be performed outdoors for group outings and to accommodate transportation needs of clients/employees. Local travel is required.
